Race No formal pharmacokinetic study was conducted in different ethnic groups. Data were gained from the population pharmacokinetic analysis where it was shown that pharmacokinetic parameters were similar in subjects of Caucasian and Hispanic origin. However, the volume of the central compartment was increased by 20% in Caucasians relative to other races. Race had no effect on clearance. No conclusion on PK in Black or Asian races could be made due to the limited number of patients. Weight Volume of distribution was correlated with weight in the population PK PD analyses. Zyrtec and Clarinex are nonformulary and aren't covered for most BCN members. But a dose of hydroxyzine Atarax Vistaril ; in the evening yields a therapeutic concentration of Zyrtec cetirizine ; the following morning. Over-the-counter Claritin Alavert loratadine ; 10mg is exactly the same as the original prescription-only version. Some members may have the notion that since it's now available over-the-counter, that somehow it's less effective than prescription-only alternatives. Please assure your patients that the over-the-counter version is identical to the original, but is just a fraction of the cost. You might also consider using the generic name loratadine 10mg ; for your members who are reluctant to consider an over-the-counter product. Extended-release forms of Claritin and Zyrtec have other ingredients that cause other symptoms, including nervousness, restlessness, and insomnia. Some patients taking Claritin-D 24 Hour Extended Release tablets have reported obstruction in the upper gastrointestinal tract, including difficulty swallowing. It is not known if this is common or typical of all second-generation agents. Drug and Food Interactions. Two earlier second generation drugs, terfenadine Seldane ; and astemizole Hismanal ; , in rare cases, caused dangerous heart rhythm abnormalities, particularly in high doses or in people who have liver disease or are taking certain other medications or ingesting grapefruit juice. Both Seldane and Hismanal have been taken off the market. Allegra, Zyrtec, and Claritin do not appear to pose any of the dangers associated with Seldane. Until more is known, anyone who takes a second-generation antihistamine, though, should probably avoid or use with caution combinations with grapefruit juice or the drugs that caused problems with Seldane and Hismanal. Such medications include the following: The antibiotics clarithromycin Biaxin ; and troleandomycin. Certain HIV protease inhibitors. The overall drug visit rate per 1, 000 population rose from 70 to 142 between 1993 and 2000, and this increase holds for all age groups. There are at least two major factors associated with this increase--dissemination of second-generation antihistamines that do not produce the sleepiness and other side-effects of previous antihistamines, and direct marketing of these drugs to consumers 120, 121 ; . The toll exacted by allergies has been greatly alleviated by nonsedating second- and third-generation antihistamines: loratadine Claritin ; , terfenadine Seldane, which was withdrawn from the market ; , Allegra fexofenadine ; , astemizole Hismanal, also now withdrawn ; , and cetirizine Zyrtec ; . The sedation related to first-generation antihistamine use has been shown to compromise performance at school and at work, impair driving, and decrease the ability to handle tasks that require a high degree of alertness or concentration. Elderly patients may be more susceptible than younger patients to the sedating and anticholinergic effects of first-generation antihistamines. Although less extensively studied in elderly patients, it is probable that second- and third-generation antihistamines are also less likely to induce the adverse central nervous system effects in older patients that are characteristic of the firstgeneration antihistamines, and this may have contributed to an increase in the utilization rate for the elderly population 122 ; . Along with the proliferation of these nonsedating drugs, direct marketing efforts are associated with increased market share. A survey of American Academy of Family Physicians concluded that prescription antihistamines and antihypertensive drugs were the drugs patients most commonly requested from their physicians 123.

Public benefit will be that natural gas utilities will have an incentive to invest in new and replacement infrastructure. The primary cost will be borne by the utilities' ratepayers in the form of both higher base rates as well as additional rate case expenses that utilities would incur in filing both the initial rate case that is required before a utility may request an interim rate adjustment, and the subsequent rate case that must be filed no less than five years after the initial interim rate adjustment. Reasonable rate case expenses are recovered from customers through a surcharge on customers' bills. Mr. Geise has also estimated that there may be a cost of compliance with the proposal for the individual, small business, or micro-business natural gas utilities because of the proposed amendment and new rule; however, because these provisions are permissive and not mandatory, there is no way to predict whether individual, small business, or micro-business natural gas utilities will utilize these provisions. Further, it is not likely that an individual, small business, or micro-business utility would find the interim rate adjustment procedures advantageous unless the utility had an active and on-going construction program. A gas utility that files an application for an interim rate adjustment must have had a full rate case at the Commission within the two years prior to filing the application; will be required to file annual adjustments to its interim rate adjustment; and will be required to file a full rate case within six months of the fifth anniversary of its initial interim rate adjustment. Finally, a gas utility implementing a tariff or rate schedule under the proposed new rule will be required to reimburse the Commission for the utility's proportionate share of the Commission's costs related to the administration of the interim rate adjustment mechanism.
