Pancreatic beta-cell function declines over time in individuals with type 2 diabetes. To quantitate the long-term success rate with oral agent therapy, we extracted from the VANCHCS computerized record the cohort of 324 veterans prescribed hypoglycemic agents every year from 7 1992 through 6 2003. Our total hypoglycemic-agent-treated population increased from 2422 to 7954 during this period, with most care being provided in a primary care setting. HbA1c was used to assess glycemic control. HPLC methodology was used from 1994 through 2003 normal range 4.1 to 6.2% ; . HbA1c values determined by affinity chromatography from 1992-1994 were converted to HPLC values. In the first year, 161 veterans received oral agents only, 118 insulin only, and 45 both therapies. Eleven years later, 112 were on oral agents only, 114 on insulin only, and 98 on both.
