In List A below remain essential uses of CFCs? Bear in mind that if you advise us that a product is no longer essential, for FDA to affect that advice, we still must hold an open public meeting and go through notice and comment rulemaking, meaning the public and other concerned parties would have the ability to weigh in on the proposed delisting. To aid in your deliberations over whether individual moieties remain essential, it is important for you to know what moieties still are marketed in CFC-containing products and listed under 21 CFR 2.125 e ; as essential, as well as what products are currently approved and or marketed that do not contain CFCs. Therefore List A below includes those moieties for which there are no current reformulations or direct alternative products and therefore are relevant to today's discussion under 21 CFR 2.125 g ; 2 ; . Other products currently listed under section 2.125 e ; may be currently unavailable for other reasons, and therefore could be delisted under alternative criteria see sections 2.125 g ; 1 ; or 2.125 g ; 3 . Also I have provided a list of potentially relevant marketed non-CFC products to help you consider whether the moieties cited in List A remain essential List B ; . LIST A. Moieties currently listed under 21 CFR 2.125 e ; for which no current reformulated or direct alternative product exists: 1. Beta-agonists * metaproterenol Alupent ; pirbuterol Maxair ; 2. Inhaled Corticosteroids flunisolide Aerobid ; , triamcinolone Azmacort ; 3. Cromones Cromolyn Intal ; Nedocromil Tilade ; 4. Albuterol Ipratropium Combivejt ; * Note that epinephrine would also be correctly included in this list. However, since this is an OTC drug product, this moiety will need to be separately considered so that the Advisors can properly include members of the Non-prescription Drugs Advisory Committee. FDA plans to hold a subsequent, timely meeting to specifically discuss epinephrine and the issue of OTC rescue bronchodilator availability. To aid in your deliberations over whether the individual moieties cited in List A remain essential under the second criteria cited in section 2.125 f ; , as discussed above, I have also provided a list of potentially relevant marketed non-CFC products List B ; . This list is provided to aid your consideration as to whether an adequate, alternative therapy is available. This list of non-CFC products does not include products for nebulization and.

Testosterone, the main circulating androgen, is converted in prostate cancer tissues by 5a-reductases into the bioactive and potent androgen, 5a-dihydrotestosterone DHT ; Russell & Wilson 1994 ; . Therefore, 5a-reductases are considered to play important roles as local regulators of androgen and other sex steroid actions in prostate cancer tissues. The type 1 isozyme of 5a-reductase is located on the distal short arm of chromosome 5, and is mainly expressed in the liver and skin Thigpen et al. 1993, Russell & Wilson 1994 ; . The type 2 isozyme of 5a-reductase is located in band p23 of chromosome 2, and is, in addition to its strong expression in the liver, also markedly expressed in the prostate, seminal vesicle, and epididymis Thigpen et al. 1992, Thigpen et al. 1993 ; . This table is designed as a guide to the selection of combivent for the correct application. Once the flue size has been calculated referring to the kilowatt rating for that diameter, will indicate the suitable combivent size. A square unit has more free area available than a circular system, therefore, will be suitable for higher appliance ratings than the equivalent circular units.

REGIONAL CHAMPIONSHIPS All Regional and National referees are required to work at least one of our Regional Championships. We welcome any of our Provisional referees who want to contribute to these Championships by working. Please contact either Tracy Shadle or Larry Liden to volunteer. Congratulations to Carla Bunner and Monica Deckers for being nominated as the Chesapeake Region's candidates for their Junior National Referee certification. Carla and Monica will be attending one of the USAV Juniors Qualifiers for their rating session. Mark Nash has been invited to the 2003 NIRSA Championships as a USA National candidate. RULES, MECHANICS, AND CLARIFICATIONS The 2002-2003 Competition Season has already created some questions about "typical" issues. Game Tempo. We want the games to flow smoothly. However, R1s first referees ; should only authorize the service when the server is in the area behind the end line with the ball in their possession. I have heard reports that our R1s are whistling for service while the next server is approaching the service zone! Let's not hurry the teams, but let's assist them in making the games matches flow at a reasonable pace. Make sure the receiving team, serving team, and work crew are ready. Wait for the server to cross behind the service line and pause facing towards or away from ; the court, with the ball in his her hands. When all these conditions are met, beckon for serve. Do not wait for the server to delay the process by getting instructions from the coach, finding another service spot, or bouncing the ball. Use of Balls Between Games. Players may use balls to pepper between games. Adults may share hitting between games by directing their hitting into their own court. Juniors may not share hitting during the between game interval -- only peppering. Libero Uniform. The Libero player's jersey shirt must be a contrasting color compared to the other team jersey shirts. The Libero player's team jersey must have a legal number on it. However, the Libero may wear another jersey, shirt, or jacket that is of contrasting color, over their team uniform such that the uniform number is obscured. It is more important to be able to.

7. Children with preexisting illness Children with certain underlying chronic medical conditions may be at increased risk of developing either an exacerbation of their chronic illness or an illness directly related to altitude, such as HAPE. Few to no data exist for determining the risk for specific medical conditions such as cystic fibrosis or chronic lung disease of prematurity bronchopulmonary dysplasia ; . However, by first possessing a knowledge of known risk factors for the development of altitude-related illnesses and then assessing how each individual child's condition may affect his or her cardiopulmonary physiology in a hypoxic environment, it may be possible to determine the relative risk of developing complications at altitude. For instance, both a relative lack of increased minute ventilation at altitude and pulmonary vascular overperfusion, such as is seen in individuals who lack a pulmonary artery, are risk factors for the development of HAPE Matsuzawa et al., 1989; Selland et al., 1993; Sebbane et al., 1997 ; . Therefore, it is logical to believe that children with congenital heart defects resulting in overperfusion of the pulmonary vascular bed, such as atrial and ventricular septal defects, unilateral absence of a pulmonary artery, and patent ductus arteriosus, would be at increased risk for the development of altitude-related illnesses like HAPE. Similarly, children who have significant lung disease secondary to premature birth or cystic fibrosis and have elevated PaCO 2 levels at sea level may not be able to increase their minute ventilation when stressed by altitude and thus be at risk for illness at altitude. Children with Down syndrome have a high incidence of both obstructive apnea and hypoventilation, as well as congenital heart defects resulting in increased pulmonary blood flow. Perhaps these physiologic abnormalities contributed to the development of HAPE in children with Down syndrome at relatively low altitudes Durmowicz, Pediatrics 2001, in press ; . Children with noncardiopulmonary disorders may also be at increased risk for the development of illness at altitude depending on how the disorder responds to the stresses of al.

Boehringer Ingelheim's respiratory portfolio con sists of major COPD and asthma products: spiriva tiotropium bromide ; , combivent ipra tropium bromide salbutamol ; and atrovent ipratropium bromide ; . spiriva is a oncedaily inhaled medicine recom mended for firstline regular treatment of COPD. It contains an anticholinergic agent which acts on airway constriction, a dominant mechanism in COPD.
