PROCEDURES THAT SUPPORT SAFE PRESCRIBING AmeriHealth utilizes a Pharmacy Benefits Management PBM ; company, FutureScripts, to manage the administration of our commercial prescription drug programs. As our PBM, FutureScripts is responsible for providing a network of participating pharmacies, administering pharmacy benefits, and providing customer service to our members and providers. Prior Authorization AmeriHealth requires prior authorization of certain covered drugs to ensure that the drug prescribed is medically necessary and appropriate and is being prescribed according to the Food and Drug Administration FDA ; guidelines. The approval criteria were developed and endorsed by the FutureScripts Pharmacy and Therapeutics Committee, which is an established group of medical directors and practicing area physicians and pharmacists. Using these approved criteria, clinical pharmacists evaluate requests for these drugs based on clinical data, information submitted by the member's prescribing physician, and the member's available prescription drug therapy history. Their review includes a determination that there are no drug interactions or contraindications, that dosing and length of therapy are appropriate, and that other drug therapies were utilized, if necessary. Without prior authorization, the member's prescription will not be covered at the retail or mailorder pharmacy. See 96-hour Temporary Supply Program on the following pages. ; The prior authorization process may take up to two working days once complete information from the prescribing physician has been received. Incomplete information will result in a delayed decision. Currently, the drugs listed below are a part of the prior authorization program. Prior authorization applies to all formulations of these specific drugs, including but not limited to, tablet, capsule, and oral suspension. * AcipHex AltabaxTM Ambien CR AmeviveTM Atacand Atacand HCT Avapro Avalide AzorTM Benicar Benicar HCT BiDil Botox Byetta Caduet Caverject Celebrex Cesamet Cialis Cozaar Hyzaar Cymbalta DaytranaTM Diovan Diovan HCT Edex Enbrel Exforge Exjade Exubera Fentora ForteoTM Gleevec GlumetzaTM Humira InvegaTM Iressa JanumetTM JanuviaTM Kineret Levitra Lipitor Lunesta Lyrica MagnacetTM Micardis Micardis HCT Mobic Muse Myobloc.

Anna Vouladakis For many years, wheelchair design and technology lagged behind that of cycling technology. In recent years, however, lighter, faster chairs have become available. These advances in chair technology, along with increased recognition for the sport, have made wheelchair racing increasingly popular and there are now over 500 racers worldwide. But a well-designed wheelchair is only one of the things an athlete needs in order to compete successfully. Proper hydration is also key to an athlete's performance. More importantly, it contributes to their overall health, as dehydration can cause fatigue, headaches, muscle weakness, dizziness, and light-headedness.and accidents. There is an abundance of hydration systems on the market for able-bodied athletes. These systems range from small backpacks to special bike-racks for water bottles. In most instances, wheelchair racers are forced to alter these products to meet their needs. Such alterations might include cutting up a hydration backpack and sewing on adhesive fabric in order to attach it to the chair; or it may involve poking holes in a water bottle so that a drinking tube can be inserted into it. For my graduation project at Emily Carr Institute, I decided to explore all aspects of wheelchair racing and to develop a hydration system, which addresses the needs of both quadriplegic and paraplegic athletes. Consideration was given to the usability, safety, maintenance, installation as well as many other factors relating to the overall performance of the product. In short, the outcome was a 750 ml reservoir that uses constant force springs to maintain consistent pressure with flow controlled by a bite valve. The pack snaps easily to a semi-permanent harness at the back of the chair, locking it into place for the next training session around the track or in preparation for an intense 42 km marathon.

2. Focus on community-level models of care that efficiently coordinate the multiple health and human service providers and public and private payers involved in mental health treatment and delivery of services. 3. Focus on those policies that maximize the utility of existing resources by increasing cost effectiveness and reducing unnecessary and burdensome regulatory barriers. 4. The Commission must consider how mental health research findings can be used most effectively to influence the delivery of services. 5. The Commission must follow the principles of federalism and ensure that its recommendations promote innovation, flexibility and accountability at all levels of government and respect the constitutional role of the United States and American Indian tribes. The first meeting was in June 2002, approximately two months after the President established the Commission and signed the executive order. We therefore started two months behind schedule. This produced significant anxiety in all of us, as we contemplated the deadlines of an interim report to the President in five months and a final report in 10 months. Most of us did not know each other, and therefore initially acted more as individuals than as a commission. Individual perspectives were aired and defended. We came from various backgrounds: six MDs; eight PhDs; one MSW; two with MBAs, including myself; three with masters' in public health; two with degrees in education; one lawyer, who is also a judge; one state senator; the Mental Health Directors of Ohio and California; and one Chairman of the Board of a large mental health managed care company. Federal departments included the National Institute of Mental Health, the Department of Education, the Veterans Administration, Centers for Medicaid and Medicare Services, Housing and Urban Development, Substance Abuse and Mental Health Services Administration and the Department of Labor. Initially we were much like blind men describing an elephant, each at times insisting that their part was the essential, or indeed, the whole part. In the first meeting we encountered problems with definitions. I challenged that we come to some consensus on definitions of the key terms and concepts before we tried to solve the problem. Indeed the scope of the problem would be determined by the definition of key terms. First, the President directed us to restrict our recommendations to seriously mentally ill adults and children with serious emotional disturbances. My questions were: How do we define serious and what diagnostic categories were we considering including? Were we only going to look at those individuals who were frequent users of mental health services, -7.

This component was included to encourage the learner to record the medical history of the patient. In addition to being part of normal practice during a consultation, documentation of the history makes explicit what the doctor or learner knows about the patient. Reflection on the information contained in the record, is a useful way of integrating that information, and directing further enquiry.

ALL FAMILY HOMEWORK EXERCISES ARE OPTIONAL First: The student has read the "Birth Control Reference Sheet 1" in class. The adult should begin by reading it, too. Next: Discuss together what your culture, your religion, and the two of you believe about: Family Size What is the ideal family size? Why? If a couple chooses not to have children, are they still a family? What is a good age to have a first child? Who is responsible for the children? Birth Control What kinds are OK, if any? Why? Are some kinds of birth control wrong, in your opinion? Which kinds? Why? Does it depend on whether a person is married? What if they are developmentally delayed? or otherwise disabled? or have a genetic disease? What if they just don't want a baby now? Whose responsibility is birth control, the man's or the woman's? Parental Consent Do you agree or disagree with the laws that say teens can get non-prescription birth control from any drug store without their parents' permission? Why? What about prescription methods from doctors? If you're a parent, would you hope your child would or would not tell you if he or she were using or wanted to use a birth control method besides abstinence? Why?.
